We are doing our part to prevent the spread of COVID-19 and have made some changes to ensure the health and safety of our staff and patients. All patients will be sent an email with the COVID-19 screening questions and consent form 24 hours prior to their appointment. A mask must be worn at all times while in the clinic.
We are limiting the amount of people in the clinic at a time and ask that you arrive for your scheduled appointment time. When you arrive for your appointment, please call or use the self check-in option before entering the clinic.
Your appointment time is reserved just for you. A late cancellation or missed visit leaves a hole in the therapists’ day that could have been filled by another patient. As such, we require 48 HOURS NOTICE for any cancellations or changes to your appointment. Patients who provide less than 48 hours notice, or miss their appointment, will be subject to a cancellation fee.
Telemedicine Appointments are offered by: Dr. Alicia Motuz, Dr. Michelle Pobega and Lindsay Wetmore. We require a credit card on file at time of booking a telemedicine appointment. Your payment information will be used to hold your appointment and in the event of a no show/late cancellation charge. Our data (JANE) is encrypted using 256 bit encryption. The same level of security as your bank uses to transfer information.
If you require supplements, we ask that you call ahead to pre-order them. Reception will prepare your order for pick up.
